How do I become a patient?
You will need a physical therapy prescription from a medical practitioner. Either the medical practitioner will refer
you or you can request to come to Fukuji & Lum. If the medical practitioner is not familiar with our clinics, we will be
more than happy to fax or mail a physical therapy prescription to his/her office. Our prescription
form can also be downloaded and
printed from our website. Qualifying medical practitioners include physicians, physician assistants and chiropractors.
How do I know if my insurance carrier will cover my physical therapy?
Please check with your employer or insurance carrier about your benefits as they vary from person to person. Also, if you
are unsure if we are providers for your insurance company, please contact us.
What do I need to do before my first visit?
- See your medical practitioner to get a prescription for physical therapy.
- Schedule an appointment for your first visit
- Download the Patient
Information Forms online and fill
them out prior to your first visit.
- Arrive 10-15 minutes early to complete the registration process. Please bring any paperwork you have already completed with you.
- If private insurance is covering your therapy, bring your
insurance card and ID. If Worker's Compensation or No Fault
is covering your therapy, bring your claim number and case
manager or adjuster’s contact information.
- Wear comfortable clothing.
